How do I renew my CA BreEZe Guard card? ›

For example if your guard card expires Dec 31, 2022, the online renewal will be available on Nov 1, 2022. To renew online, go to the CA State License Registration website, at https://www.breeze.ca.gov and log in. If you don't already have a Breeze account, you will need to create one, and then log in.

All applicants should check for status updates by checking their BreEZe account or visiting our Application Status and Details page. Applicants may also use the contact us page, or call the BRN using the automated phone system at 916-322-3350.

If you prefer to remain anonymous, you may still file a complaint by calling the Department of Health Complaint Hotline at 800-792-9770. The hotline is available 24 hours a day.

You can file a complaint directly with the Medical Board 24/7 through the confidential complaint hotline at 1-833-333-SMBO (7626) or online at med.ohio.gov. Provisions in the Ohio Revised Code make all complaints received by the board confidential.

The California guard card can be renewed if it has not been expired for more than 60 days with a payment of $65.00. If your guard card has been expired for more than 60 days you would need to repeat the entire guard training and licensing process before being issued a new permit.

You can also monitor your balance by phone if you're not able to check online. Have your card ready and call 404-848-5000 to reach our automated answering service. Every Breeze Vending Machine and MARTA RideStore gives you the option of checking the balance on your current Breeze Card.

If the investigation finds evidence that the physician has violated the Medical Practice Act and the violation warrants disciplinary action, the case is forwarded to the Attorney General's Office to prepare an accusation.

Report directly to the state licensing board when the conduct in question poses an immediate threat to the health and safety of patients or violates state licensing provisions. Report to a higher authority if the conduct continues unchanged despite initial reporting.

Complaint Requirements

Your complaint must: Be filed in writing by mail, fax, e-mail, or via the OCR Complaint Portal. Name the covered entity or business associate involved, and describe the acts or omissions, you believed violated the requirements of the Privacy, Security, or Breach Notification Rules.

Your license is valid for two years and expires the day after the expiration date that appears on your card. It's recommended that you submit your guard card renewal application to BSIS at least 60 days prior to the expiration date but no sooner than 90 days.

Go to the TSA PreCheck website, select the tab that says “Renewal,” and enter your Known Traveler Number, date of birth and legal last name into the appropriate fields.
